If you are on regular medication you will need to know how to obtain your prescription each month.
Wherever possible please bring in or post your white printed form (this should have been given to you at the Pharmacy or Surgery).
If you do not have possession of a white printed form then please let us have written details of your prescriptions request giving the following information:

The name of the item or items you require.
